The CHRONIC DISEASE SELF-MANAGEMENT PROGRAM is designed to learn problem-solving, decision-making, and other techniques for managing problems common to people with chronic diseases CDSMP is for adults with chronic health conditions such as arthritis, diabetes, heart disease, lung disease, and other ongoing health problems.
Diabetes Self-Management
This is a community-based workshop to help people manage their diabetes. This workshop is recommended for adults with Type 2 diabetes or who are pre-diabetic. This interactive workshop is designed to improve individual’s A1C’s and reduce the complications associated with diabetes.
Tai Ji Quan: Moving for Better Balance®
This exercise routine program contains a set of Tai Ji Quan-based individual forms and movements that have been transformed into therapeutic applications for training ankle stability, effective weight transfer, active eye-head movement, and spatial orientation, as well as enhancing skills directly transferable to daily functional activities such as reaching, sit-to-stand (stand-to-sit), stepping and turning, and walking.
The goal of these exercises is to adapt and integrate sensorimotor systems, refine postural control and movement strategies, improve gait and locomotion, strengthen lower-extremity muscles, and increase flexibility.
Opioid/Prescription Drug Misuse
Opioid/Prescription Drug Misuse program provides vital education and support. Tailored for seniors, we address the unique challenges and risks associated with opioid and prescription drug misuse. Through targeted outreach, resources, and community engagement, we empower seniors to make informed choices, fostering a safer and healthier aging community.
PEARLS Counseling Service
The Program to Encourage Active and Rewarding Lives (PEARLS) is national evidence -based program designed to reduce depression in physically impaired and socially isolated individuals. PEARLs offer empower older adults by teaching them skills necessary to take action and make lasting life changes so they can lead more active rewarding lives.
